Pharmacological properties

The drug belongs to the group of combined drugs that have an antihypertensive effect. The combination of active ingredients allows you to achieve a more pronounced therapeutic effect than their use alone. The first main ingredient (telmisartan) is an angiotensin-2 receptor antagonist. At the same time, it does not suppress the activity of these receptors and has no structural similarity with other binding receptors.

Telsartan H 40-80 mg. Instructions for use, price, reviews

Taking the drug leads to a decrease in the level of aldosterone, does not suppress the effect of renin and does not cause blocking of ion channels. The component does not reduce the activity and action of the angiotensin-converting enzyme. The drug promotes the destruction of bradykinin and leads to a decrease in all risks of developing side effects caused by the substance.

When the 80 mg tablets are taken, there is a decrease in blood pressure caused by angiotensin-2. The therapeutic effect is noted after 3 hours after taking the medicine. In this case, the effect lasts for a day. A pronounced therapeutic effect occurs 1 month after the systematic use of the drug.

The active substance helps to lower blood pressure, but does not affect the heart rate (HR). After discontinuation of therapy, the pressure gradually returned to baseline values. In this case, the pseudosyndrome does not develop. The second main ingredient (hydrochlorothiazide) is a representative thiazide diuretic. Its action is aimed at expanding arterioles, thereby reducing pressure. Under normal pressure, the substance does not affect.

The ingredient is able to remove electrolytes and fluids. However, it begins to act 2 hours after application. The maximum efficiency is noted after 3-6 hours. The result lasts for 6-12 hours. After discontinuation of the drug, the effect lasts for about 1 week. To achieve a hypotensive effect, lower dosages are required than for the development of a diuretic effect.

When using high single dosages, there is a rapid development of the antihypertensive properties of the drug. When using small doses, blood functions remain little changed. The active ingredient can lead to disturbances in the production of breast milk. The maximum effect of the drug is achieved with its systematic intake for 1-2 months.

Pharmacodynamics and pharmacokinetics

Telsartan N 40 mg (the price of the drug has nothing to do with its pharmacological properties) and 80 mg have identical mechanisms of action. The first active ingredient is well absorbed from the gastrointestinal tract. The bioavailability level is about 50%. The maximum concentration is reached after 0.5-1.5 hours. Food intake can reduce this indicator, but after 3 hours. concentration is balanced.

The level of protein binding is above 99.5%. The ingredient lends itself to metabolic transformation and breaks down into inactive forms. The half-life of the main substance is over 20 hours. It leaves the body in an unchanged form through the intestines. In this case, only about 2% is excreted by the renal system. In patients with renal insufficiency, bioavailability reaches 100%. In this case, the half-life remains unchanged.

The second active component interacts with proteins. At the same time, the level of interaction varies from 40 to 70%. Bioavailability is about 60%. The maximum concentration of the ingredient occurs within 1-3 hours after application.

The main substance does not lend itself to metabolic transformation and leaves the body in an unchanged form along with urine. Approximately 60% of the taken dosage is excreted after 2 days. The half-life is from 12 to 15 hours. With functional impairment of the kidneys, the elimination time may be reduced.
