Periarthritis of the shoulder joint( PPS) is a disease of the shoulder joint, associated with inflammatory changes of the periarticular tissues.
Reasons for
The causes of PPP are varied. These can be injuries of the joint and surrounding tissues, inflammatory processes in it, increased stress( microtraction) on the joint, excessive sport craze associated with monotonous movements in the shoulder joints( for example, bodybuilding). This pathological condition can also lead to anatomical features of the structure of the joint.
Epidemiology and prevalence of
The disease is equally common in both men and women. This disease accounts for one-tenth of all orthopedic diseases.
Symptoms of the periarthritis of the shoulder joint
At the very beginning of the disease, minor pains appear in the shoulder joint, which are of a non-permanent nature. These pains can intensify with certain movements, with the arm rising above shoulder level, with the turn of the shoulder or with the arm behind the waist. In rest, however, no sensations may exist and be so insignificant that a person will not pay attention to them. Therefore, at this stage, a small number of people turn to medical help.
In the future the disease progresses, and then the pains become more intense and permanent. They begin to disturb at rest and even in a dream. Pain can be given to the scapula and neck. It prevents a person from working and resting. At this stage, the patient begins to pay more attention to their health and goes to the doctor.
During the examination, the doctor can find signs of inflammation in the shoulder joint in the patient: a slight soreness on palpation, swelling and, possibly, redness. As for the examination, a decrease in the amplitude of movements in the joint will appear. The patient can not get a hand on the lower back. But with all this, the swaying movements in the hand parallel to the trunk will be preserved.
A crunch can appear in the joint during movements.
If the disease began a long time and the patient delayed a visit to the doctor about this, then atrophy of the periarticular muscles, primarily deltoid, is possible. And in the bones that form the joint, the appearance of osteoporosis is possible. All this is associated with a decrease in the load on the damaged joint and its muscular apparatus.
Diagnosis of the periarthritis of the shoulder joint
Radiological examination is crucial in the diagnosis of periarthritis of the shoulder joint. Pictures will be taken in 2 projections. They can reveal changes in the bones of the shoulder joint, changes in the tubercle of the humerus, various calcareous deposits.
Treatment of periarthritis of the shoulder joint
In case of exacerbation of the disease and with severe pain, symptomatic treatment is used. For this purpose, non-steroidal anti-inflammatory drugs are prescribed, such as: paracetamol, diclofenac, ibuprofen, nimesulide, aspirin. You can also use physiotherapy, in the form of electrophoresis with drugs on the area of the affected joint. In unbearable pain, periartrikular blockades with novocaine and hydrocortisone( glucocorticoids) are used. The joint should not be fixed, as this can lead to a permanent loss of the amplitude of its movements.
Outside the exacerbation, it is recommended to perform joint exercises and spare the affected joint for various types of work, especially those associated with raising the arm above shoulder level. If this is not possible, you must change the work.
Forecast of the disease
With a correct and long-term treatment and compliance with medical recommendations, the prognosis of the disease is favorable. Otherwise, there may be a persistent loss of movement in the joint, as a consequence of the progression of the disease, eventually in the event of disability.
