Composition and form of release
Nicergoline has two forms of releasewith different composition:
- Round two-layer tablets .One tablet contains the following ingredients: active ingredient nicergoline( 10 mg) and additional elements - 70 mg lactose monohydrate, 1.1 mg magnesium stearate, 2 mg pyrogenic silicon dioxide and 26.9 mg potato starch.
- Lyophilizate for injection solutions .It looks like a white powder in glass ampoules. One such ampoule contains 4 mg of nicergoline and additional components: tartaric acid( 1.04 mg) and lactose( 30 mg).To make the solution, a solvent( 0.9% sodium chloride solution) is also used.
Pharmacological profile
The drug improves hemodynamics of the brain and activates its metabolic processes. It is able to suppress the rebound of cerebral vessels and increase the consumption of glucose and oxygen by brain tissues. The constituent components of the drug accelerate the flow of blood through the vessels of the limbs. In the presence of a constant elevated pressure, the drug contributes to its smooth decrease.
The nicotinic acid particles that are contained in the drug have a direct spasmolytic myotropic effect on the 
myotonic walls of resistive vessels, thereby increasing their throughput.
The bioavailability of the drug is 60%.The largest accumulation in the blood is observed one hour after ingestion.
The process of metabolism takes place in the form of hydrolysis, glucuronization and dimethylation reactions( 90% of the drug is metabolized).
Nicergoline and its metabolites can be distributed in blood cells. The half-life of all components of the drug is two and a half hours. Close to 75-80% of metabolites are excreted by the kidneys( 80-100 hours after ingestion).20% of the drug is excreted with feces.
The mechanism of action is to provide a vasodilator effect that occurs against the background of blocking calcium channels and adrenoblocking action. Also, the drug is able to have an antispasmodic effect on peripheral vessels.
Indications and contraindications for use
The drug is indicated for its intended use in the following situations:
- acute and systematic disorders of cerebral blood flow;
- vascular disorders of the peripheral circulatory system;
- presence of obliterating endarteritis;
- violation of vascular genesis( labyrinth and vestibular);
- increase in the volume of the prostate;
- infarction of the optic disc;
- migraine and vertigo;
- presence of dystrophic diseases of the cornea;
- vascular complications of diabetes mellitus;
- atherosclerosis of the arteries of the extremities;
- systematic increase in blood pressure( in this case, Nicergoline is used as an auxiliary therapy).
Contraindications to prescription:
- previously suffered myocardial infarction;
- acute susceptibility to the constituent elements of the drug;
- manifestation of a rare heart rhythm;
- incorrect orthostatic regulation;
- presence of acute bleeding;
- pregnancy and lactation;
- presence of organic heart lesions;
- for children( under 18) and elderly( after 65 years) age;
- dystrophic changes in the liver.
Instruction for use
The method of taking the medicine depends on the form of its release. Tablets are provided for internal administration. The amount of the drug at one time is 10 mg, with a frequency of three times a day. In most cases, the therapeutic course lasts for several months.
In the treatment of vascular dementia, 30 mg of the drug is prescribed, with a frequency of reception twice a day. In the case of functional disorders of the kidneys, the dose of the drug should be reduced. The injections of Nicergoline are given intravenously, intramuscularly and intraarterially. Pre-diluted with a solution of the essence is introduced in an amount of 2-4 mg, twice a day. The minimum length of the therapeutic course is two months.
This is due to the slow development of the therapeutic effect of the drug.
Overdose and Adverse Reactions
The main signs of overdose are a rapid lowering of blood pressure, orthostatic collapse and the manifestation of allergic 
reactions.
If the therapeutic dosage is exceeded, there is an increase in adverse reactions. When these symptoms occur, symptomatic therapy is necessary, since there is no special antidote.
Side effects include dizziness, sleep disturbances, and the appearance of a hypotonic condition. Disturbance of digestion and hyperemia of the face and trunk is also possible.
Special instructions
After iv introduction of the drug, orthostatic hypotension may develop. To avoid this situation, the patient should take a horizontal position for 15-20 minutes. The use of alcoholic beverages during treatment is strictly prohibited. The drug affects psychomotor reactions and the ability to concentrate attention.
Medication is not recommended for prescription in childhood. The ability of the components of the drug to enter the breast milk is not confirmed, so it is forbidden to take it to nursing mothers.
During pregnancy, medication is prescribed only if absolutely necessary and after complete examination of the patient. Patients with renal insufficiency should be reduced in the dose of the drug.
Drug Interactions
Nicergoline increases the effects of tranquilizers, neuroleptics and hypotonic drugs. When combined with antacids, the interval between use should be at least two hours.
Joint reception with antihypertensive drugs enhances the effect of the latter. Also, Nicergoline increases the effect of antiaggregants and indirect anticoagulants. It is not recommended to take in parallel with the drugs that excite the central nervous system.
When combined with cholestyramine, Absorption of Nicergoline is slowed.
